PATIENT FINANCIAL POLICY

Thank you for choosing the Michael Horn Center for Cosmetic Surgery! We are committed to the success of your medical treatment and care. Please understand that payment of your bill is part of this treatment and care.

For your convenience, we have answered a variety of commonly asked financial policy questions below. If you need any further information about any of these policies, please ask to speak with a staff member for additional assistance.

Is there a fee for the consultation?
No. Our consultations are complimentary. We take pride in knowing that our patients schedule surgery based on the knowledge and comfort that we provide them from their first phone call, all the way through their surgical experience. We do not want our patients to feel bound by any fee that they would incur just to learn about Dr. Michael Horn and his expert surgical technique.

If a patient must reschedule a consultation appointment, the patient must give notice 48 hours before the consultation takes place. There is a $150 no-call, no-show fee that will be applied if the patient does not give a 48 hour notice of cancellation.

How may I pay?
We accept payment by cash, Visa, MasterCard, American Express, Discover, cashier's check and money order. Our office does not accept personal checks. There are four different financing companies that we use on a daily basis. These companies can approve a cosmetic loan right over the phone in a matter of minutes! The names of those financing companies are as follows:

If you are approved for a loan through one of these companies, they will provide you with a payment program according to their specific rates and the amount that you are approved for.

When is payment due for my surgery?
The surgery fee must be paid in full before the surgery takes place. This payment will be due in full on the scheduled pre-op date indicated for each individual patient. This date generally takes place two weeks before the actual surgery, but no later than one week prior to scheduled surgery. If the patient is financing surgery, the loan must be approved, and all paperwork must be completed on or before the pre-op takes place in order to reserve the actual date of surgery.

Is there a surgery deposit?
Yes. Once a patient decides to have surgery on a specified date, we will collect a $500 non-refundable scheduling and booking fee in order to reserve that specific scheduled date. This will be deducted from the total cost of the surgery. Patients may reschedule their date of surgery any time before the scheduled pre-op date. If the surgery is cancelled or rescheduled after the pre-op date but more than 24 hours before the surgery, the surgery fee will be refunded minus the deposit. In the event that a patient cancels less than 24 hours before the scheduled surgery time, the office will retain half of the procedure price for administrative fees. Should this happen more than once, the office will retain $500 for each additional cancellation made in fewer than 24 hours before the surgery. Nonetheless, the patient can apply any non-refundable deposits or fees to their next surgical procedures performed by Dr. Horn.

If the surgery is cancelled the day of the surgery by Dr. Horn or by the Surgery Center due to medical reasons or risk concerns, the patient is still responsible for the payment of the facility and anesthesia fee of $750. In this case the surgery fee will be refunded minus $750. Should Dr. Horn decide that the surgery can take place safely at a later time, the fee will be waved.

What does my price quote for surgery include, and how long will it remain in affect?

Patients will receive a documented price quote for surgery at their initial consultation. These prices include the following:

• Facility fee
• Anesthesia fee
• Surgeon's fee
• Garments
• Pre and post-op care

The quoted price does include the general pre-operative testing performed at the surgery center the morning of the surgery. The testing includes a finger stick to check for hemoglobin level and for female patients a urine specimen will be collected to check for pregnancy. The price does not include any additional testing required by Dr. Horn before the surgery day. These tests may include a complete blood count, EKG, and also a clearance letter from the primary care physician and will vary depending on the patient's health, medical history, and age. The payment for these additional tests is solely the responsibility of the patient and the results must be received by our office no later than three days prior to the surgery. Dr. Horn or his nurse will discuss the need for any additional testing either during the consultation or at the pre-operative appointment.

There is a specified drug regimen that patients must follow that will not be included in the price quote. These prescription medications can be covered with personalized health insurance. If the patient does not wish to use health insurance, these medications can be purchased at any pharmacy for a total of about $85 dollars. Our prices are very reasonable and competitive and will therefore be honored for 3 months past the date that the actual quote is given.
